We need some information to grant you a MyChart account. After you submit this request, it may take up to a week for processing and verification. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. Enter your demographics here making sure you use the email address you have on file at your provider's office. In the next step, you will be asked a series of questions to authenticate your identity. UHS contracts with a third party vendor (Experian) for the authentication process. UHS does NOT have access to any of this personal information. Once verified, you will receive an email or letter with your activation code and instructions on how to activate your MyChart account. If you receive a failure message, please email MyChart Support at mychart@nyuhs.org or call 607-584-4004.
